Sometimes you may need extra care to assist in your recuperation after you’ve been discharged from hospital. If this is the case, you’ll be put on a post acute care program, where the hospital engages home nursing services from RDNS to look after you at home and make sure your recovery is smooth.
Our nursing staff can help you by:
- monitoring your post-operative recovery
- attending wound care
- helping you manage severe pain
- providing general nursing care
- providing personal care.
To be put on a post acute care program, you must be a public hospital patient and assessed as needing healthcare that can’t be provided by an alternative community healthcare provider at that point in time.
How can I access this service?
It is the role of the hospital staff caring for you to make all the arrangements for post acute care services. Plans are usually started a few days before you check out of hospital, and equipment and medications for the following days may be sent home with you.
