What is it?
If you are a Royal District Nursing Service client in hospital, or needing RDNS services when you go home, one of our Clinical Nurse Consultant Liaisons can work with hospital staff to make sure your transition from hospital to home goes smoothly. They are based in the major Melbourne hospitals and act as a link between the hospital and your home.
Our Clinical Nurse Consultant Liaison can help you by:
- Identifying that you might benefit from RDNS services when you leave hospital;
- Making sure that the RDNS nurses looking after you have the information and equipment they need to help you meet your healthcare goals;
- Assessing your needs while you are in hospital so that suitable plans can be made for your discharge;
- Helping to plan your ongoing healthcare requirements with the hospital, you and your carers and other community service providers; and
- Facilitating information sharing between your home and the hospital.
How can I access this service?
If you are an RDNS client going into hospital, ask your primary nurse to talk with the District Nurse - Liaison. When in hospital, ask the hospital staff to arrange for the District Nurse - Liaison to visit you and the ward staff.
