What is it?

Before providing health care, it is important to assess your health needs and develop a care plan. When several health care providers are involved in your care, a case manager can be valuable. A case manager acts as a central contact point for everyone involved in your care and as an advocate for you, to make sure all your needs are met by the most appropriate carer. A case manager works as part of your healthcare team and can be your nurse, your general practitioner or another healthcare provider.

Our nurses can help you by:

  • Assessing your health needs and helping you develop a care plan;
  • Arranging specialist assessment if required;
  • Helping you find and link with other community services if required; and
  • Working in partnership with other health service providers to ensure optimal care is coordinated and monitored.

Which staff can help?

All our nurses are skilled in general nursing assessment and case management. If you have health needs that require specialist nursing assessment, our nurses can arrange that through one of our specialist nurses. Alternatively, we will refer you to another healthcare provider as appropriate.
