Hospital Admission Risk Program (HARP) projects are State Government funded initiatives that form the preventative arm of the Hospital Demand Management (HDM) strategy. The aim of the strategy is to strengthen the capacity of the health system to manage increasing emergency demand pressures within the public hospital sector.
HARP projects aim to reduce the demand for public hospital services by
developing preventative models of care that involve both hospital and the community. These initiatives focus on people who have chronic or complex conditions, and on the services required by high volume or frequent users of the acute public hospital system.

RDNS is involved in a number of separately funded HARP projects across metropolitan Melbourne. The projects involve RDNS working closely with hospitals and community service providers including General Practice Divisions, Primary Care Partnerships and Community Health Services.
