What is it?
Sometimes you need additional support to assist in your recuperation following discharge from hospital. You may be discharged on a Post Acute Care program, where the hospital purchases home nursing services from RDNS to attend your care on a fee-for-service basis.
Our nursing staff can help you by:
- Monitoring your post operative recovery
- Attending wound care
- Helping you manage severe pain
- Providing general nursing care
- Providing personal care.
Which staff can help?
All our nurses have skills in general and technical nursing, and are supported by expert nurses known as Clinical Nurse Consultants. These specialist nurses work alongside our general staff to ensure that your entire healthcare needs are met.
Client criteria for Post Acute Care programs
There are certain conditions that need to be met for you to become a post acute care client.
You must be a public hospital patient and assessed as requiring services that at the point of referral cannot be provided by an alternative community healthcare provider.
How can I access this service?
It is the role of the hospital staff caring for you to make all the arrangements for post acute care services. Plans are usually started several days before your discharge, and equipment and medications for the next few days may be sent home with you.
